A 35-year-old male presents to the outpatient department with high-grade fever, severe headache, retro-orbital pain, and generalized myalgia of 3 days duration. On examination, his temperature is 39.2 C, pulse rate is 64/min, and he has a positive tourniquet test with petechiae over the forearm. Which of the following laboratory investigations is the most sensitive and reliable indicator for early diagnosis of this condition within the first 5 days of symptom onset?
- A.Serum IgM ELISA
- B.Serum IgG ELISA
- C.NS1 Antigen detection testCorrect answer
- D.Peripheral blood smear for microfilariae
Solution
The clinical presentation is characteristic of Dengue fever. The Non-Structural protein 1 (NS1) antigen is detectable in high concentrations in the serum of dengue-infected patients from day 1 up to day 5 to 9 after the onset of fever, making it the most sensitive test for early diagnosis. IgM antibodies usually appear by day 5-7 of illness.
