A 58-year-old man with chronic heart failure and reduced ejection fraction is initiated on sacubitril-valsartan. Which of the following is the primary mechanism of action of sacubitril?

  1. A.Direct renin inhibition
  2. B.Neprilysin inhibitionCorrect answer
  3. C.Aldosterone receptor antagonism
  4. D.Angiotensin receptor blockade

Solution

Sacubitril is a neprilysin inhibitor. Neprilysin is the enzyme responsible for the degradation of vasoactive peptides such as natriuretic peptides, bradykinin, and adrenomedullin. By inhibiting neprilysin, sacubitril increases the levels of these peptides, leading to natriuresis, vasodilation, and inhibition of fibrosis and remodeling. Valsartan is the angiotensin II receptor blocker component of the combination.
